"I'm self-pay"
At the End of the Day, You’re Covered
Paying cash for medical care is always your choice. Even if you have health insurance, you are allowed to ask for the cash pay price of every procedure.

STEP
Save Your Guardianship Team Phone Number
Contact your Guardianship Team at the number shown on your Apta Guardianship Member Card to schedule your appointment.

Up to 95% of your care can be handled by our Guardianship Team. Care that can be resolved is covered at no additional cost to you.

Most of the time you will only need to go through Step 1. If additional, in-person care outside of your Guardianship Team is needed, you will receive instructions on how to proceed.

STEP
Confirm and
Get Ready
After working with your Guardianship Team to schedule an appointment with a third-party provider, confirm the “self-pay price” for services. Once the price is confirmed, contact your Guardianship Team and they will instruct you how to handle payment. Most of the time, it’s with your Cash Pay Card or a pre-paid voucher.

At Check-In,
Say "I'm self-pay"
When you arrive for your appointment tell the front desk: “I’m a self-pay patient.”
Pro tip: if there’s a form, just write “self-pay” where it asks about insurance. Easy win.

Bring your Apta Guardianship Member Card & Cash Pay Card
Bring your Apta Guardianship Member Card and Cash Pay Card. Do not present any other ID cards, as it may cause duplicate billing.

STEP
Pay at Your
Appointment
Pay however your Guardianship Team instructed you to. Usually this is done with your Cash Pay Card or a pre-paid voucher.

​Ask the front desk for an itemized receipt.
You’ll need this for step 5.

Send Us
the Receipt
After your visit, grab that itemized receipt (the detailed one that shows what you paid for with each member’s name).
Then:
Take a picture and text it to the number on your Apta Guardianship Member Card or
Email it to cashpay@apta-health.com.
